The third one is at the other end of the adjusting bar, loosening it allows it to pivot 
slightly.

I take of the wheel and do it from the wheelwell. I also do the inner CVs this way, 
using a really long extension... I think everyone has pretty much covered all the 
possibilities, including numerous interesting terms for the indexing protrusion on the 
crank pulley! ;-)
